What do I have to do to qualify?
Produce at least $25,000 in gross/paid premium during 2007. 2nd quarter and beyond must be paid placed premium. 1st quarter is paid during 2nd quarter, etc.

How do I qualify?
All collected new business premium from approved and placed LTC applications dated and submitted to APN after January 1, 2007 through December 31, 2007 count toward the bonus.

How much is the bonus?

Paid placed traditional LTCI premium of the following:
$1.00 - $24,999.00 No Bonus
$25,000.00 - $49,999.00 $25/application
$50,000.00 - $74,999.00 $50/application
$75,000.00 - $99,999.00 $75/application
$100,000.00 - $124,999.00 $100/application
$125,000.00 - $149,999.00 $125/application
$150,000.00 - $174,999.00 $150/application
$175,000.00 - $199,999.00 $175/application
$200,000.00 and over $200/application

When will the money be paid?
Quarterly beginning April 15, 2008.
